Stretch bar slipped and tore the tack strip right out of the floor

Last Tuesday in a townhouse off Route 9, I was doing a living room redo and power stretching a piece of berber. The bar caught a seam and popped, yanking two rows of tack strip clean out of the subfloor with a loud crack. Homeowner heard it from the kitchen and came out asking if that was normal. I had to pull the whole roll back, re-nail the strip with 1 inch pins, and redo the stretch from the far wall. Took an extra 40 minutes and I was sweating the whole time. Has anyone else had a stretch bar fail mid-pull like that, or was mine just worn out?
