Air hose got me in the face at 40 feet, changed my whole pre-dive checklist

Was doing hull cleaning in Norfolk last Tuesday when a worn O-ring blew on my second stage, I panicked, went up too fast, and sat in the chamber for 4 hours, anyone else had a near miss that made them redo their gear checks from scratch?

Funny you mention O-rings, my buddy once lost a fin strap mid-descent and we spent twenty minutes chasing it like a sea monster.
