Med records with typos got my claim flagged twice

Denied twice before I noticed the doctor's note said 'chronic back paint' instead of 'pain.' My adjuster in Fresno told me that one typo made the file look inconsistent. Pulled every record and checked names, dates, and diagnoses line by line. Took me a whole weekend but the third submission went through in 3 weeks. Has anyone else caught errors like that in their paperwork?

Wait, how did I never think to check for stuff like that? I found "diabetic" spelled wrong in my own records and my pharmacy had been giving me the wrong insulin dose for months before I caught it.
