The SSA lost my MRI report twice, now I'm stuck in a 8 month vortex

Filed my claim back in March, sent my lumbar MRI from Kaiser in Oakland with the initial packet. Last month they said they never got it, so I faxed it again with a cover sheet. Called yesterday, and the rep told me they need the interpreting radiologist's signed addendum, which my doctor only does in person. I booked the follow up for next Tuesday, but my 45 day decision window already passed, so I'm just counting lost weeks now. Has anyone else had their records vanish between the fax machine and the file room, and what did you do to jog things along?

Did you get a fax confirmation page when you sent it? That paper trail is the only thing that got mine moving after they "lost" a discogram twice. Bring that plus the signed addendum to your next appointment and ask for a copy right there in the office before you leave.
