I asked a nurse why she double-checked my arm band, and she gave me a 30-second answer I keep thinking about
Last Tuesday I was sitting in a clinic in Boise waiting for a blood draw, and I asked the nurse why she looked at my arm band 3 times before touching the needle. She said a woman with the same last name as another patient got the wrong label on a tube once, and the lab caught it 2 days later. She said near-misses like that get reported even when nothing bad happens, and the report is what changes the steps. I always thought mistakes came from people not caring, but she said it's usually a system that let two things look the same. Now I ask more questions when I'm in a waiting room instead of just sitting quiet. Has anyone else had a worker explain a small habit that turned out to be a whole safety thing?
