My whole QA process fell apart last Tuesday on a $12k commercial build

We had a pipe burst in a downtown office tower because I trusted my new guy's pressure test instead of double-checking it myself. I spent the whole day ripping out drywall and redoing joints, and it cost me two full days of labor. What do you do when a shortcut you let slide turns into a full blown disaster on site?
